Terms of Sale/Service for Dental Practice

1. Appointment Cancellation and No-Show Policy:

1.1. For all appointments, patients are required to provide a minimum of 48 hours' notice for cancellations.

1.2. Cancellations made less than 48 hours before the scheduled appointment will result in a £35 late cancellation fee.

1.3. Failure to attend a scheduled appointment without prior notice will result in the loss of the deposit made for that appointment.

2. Deposit Policy:

2.1. A 50% deposit is required for all treatments at the time of booking.

2.2. New patient exams also require a 50% deposit at the time of booking.

3. General Terms:

3.1. All fees are payable in full at the time of treatment, unless otherwise specified in these terms.

3.2. Prices are subject to change without prior notice.

3.3. We reserve the right to refuse or terminate treatment if a patient is disrespectful, non-compliant, or fails to adhere to our policies.

4. NHS Patients:

4.1. NHS patients must pay the applicable band charge before they can book their treatment.

4.2. Failure to pay the required band charge may result in the inability to schedule or receive treatment.

4.3. NHS patients not seen for over 2 years will be taken off NHS book.

5. Emergency Patients:

5.1. Emergency patients must pay 100% of the treatment fee over the phone at the time of booking.

5.2. If an emergency patient fails to attend the scheduled appointment, they will lose the entire payment made.

6. Privacy and Confidentiality:

6.1. All patient information and records are treated with the utmost confidentiality in accordance with applicable privacy laws.

6.2. Patients' personal and medical information will not be disclosed to third parties without explicit consent, except as required by law.

7. Payment and Insurance:

7.1. Payment for services not covered by insurance is due at the time of treatment.

7.2. Patients are responsible for verifying their insurance coverage and understanding the extent of coverage for specific treatments.

7.3. Insurance claims will be submitted on behalf of the patient, but any outstanding balances are the responsibility of the patient.

8. Treatment Plans and Informed Consent:

8.1. A detailed treatment plan, including estimated costs, will be provided before initiating any major treatment.

8.2. Patients are encouraged to ask questions and seek clarification on proposed treatments before providing informed consent.

9. Emergency Procedures:

9.1. In the case of a dental or medical emergency during non-business hours, patients should follow the provided emergency contact procedures.

9.2. Emergency services provided outside regular business hours may be subject to additional charges.

10. Minors and Guardianship:

10.1. Parents or legal guardians are responsible for providing consent for treatment of minors.

10.2. Parents or legal guardians are responsible for accompanying minors to appointments and providing necessary information.

11. Follow-up Appointments:

11.1. Follow-up appointments recommended by the healthcare provider should be scheduled and attended to ensure optimal health outcomes.

11.2. Failure to attend follow-up appointments may impact the success of treatment.

12. Code of Conduct:

12.1. Patients are expected to behave respectfully towards staff and other patients.

12.2. Any disruptive or disrespectful behavior may result in the termination of services.

13. Advanced Notice for Specialised Services:

13.1. For specialised services or procedures, advanced notice or preparation may be required from the patient.

13.2. Failure to follow pre-procedure instructions may result in the rescheduling of the appointment.

14. Communication Preferences:

14.1. Patients may specify their preferred method of communication for appointment reminders, billing inquiries, and other correspondence.

14.2. It is the responsibility of the patient to inform the practice of any changes in contact information.

15. Refund Policy:

15.1. Deposits and payments made are non-refundable unless otherwise determined by the dental practice.

15.2. Refunds, if applicable, will be processed within a reasonable timeframe.

By booking an appointment with our dental practice, you acknowledge and agree to abide by these terms of sale/service. The dental practice reserves the right to modify these terms without prior notice. Patients will be notified of any changes to the terms as soon as possible.
